Terms of Service Agreement
AUTOMATIC RENEWAL & RECURRING BILLING NOTICE
Enrollment in the training program authorizes Performance Ready Athletics, LLC to initiate automated monthly payments using the payment method provided. Billing will recur every thirty (30) days from the Client’s official start date and will continue indefinitely until a formal cancellation request is submitted in accordance with the cancellation policy outlined below.
Performance Ready operates as an automatic withdrawal service, and it is the responsibility of the Client to manage their enrollment status.
CANCELLATION POLICY
Clients may cancel after the first 30 days with written notice submitted at least 7 days prior to the next billing date. In addition to providing written notice, clients are required to log in to their account on the website and complete the cancellation process directly through the platform. It is the client’s sole responsibility to ensure that the cancellation is properly completed. Failure to complete both steps will result in continued billing, and no refunds or credits will be issued for charges incurred.
NO REFUNDS | FINAL SALE POLICY
All sales are final and non-refundable. No credits, rollovers, or prorated adjustments will be granted for missed sessions, early cancellation, or dissatisfaction with results.
MODIFICATIONS POLICY
All training plans are fixed and non-transferable. No substitutions, swaps, or rescheduling of sessions will be permitted.
SESSION EXPIRATION AND NON-ROLLOVER POLICY
All sessions must be used within thirty (30) days of the Client’s official start date. Unused sessions expire at the end of each monthly billing cycle and do not carry over. There are no exceptions, including absences due to illness, travel, or personal scheduling conflicts.
RECOMMENDED COMMITMENT PERIOD
Clients are strongly encouraged to commit to a minimum of six (6) consecutive months of participation in the training program. This recommended timeframe is based on the professional judgment of the trainer and is designed to optimize client results, establish routine, and maintain program consistency.
While a six-month term is not contractually required, Clients should understand that short-term participation may significantly reduce the effectiveness of the program. Clients seeking sustained results and full value are advised to engage in consistent training over this preferred period.
CLIENT ACCOUNTABILITY AND PARTICIPATION
This program is intended for individuals who are ready to commit to their health and fitness goals. The trainer invests time and resources into each custom plan, and sporadic or short-term participation is strongly discouraged.
Clients who discontinue and later wish to rejoin may not be guaranteed placement. Re-enrollment is subject to availability and may be offered at updated pricing.

Friday
5 am | 6 am | 7 am | 8 am
Clients will be assigned a specific time slot and are expected to attend only that designated session. For information regarding the private training schedule, please contact us.
GYM MEMBERSHIP FEE
A separate $55 monthly gym membership fee is required and must be paid directly to Madhouse Gym. This fee is not included in your training package. Madhouse Gym offers 24/7 access under its own terms and policies.
LIMITATION OF LIABILITY
By enrolling in the program, the Client releases Jonah Amos and Performance Ready Athletics, LLC, as well as any employees, contractors, or affiliates, from liability related to payment, training outcomes, session availability, or program effectiveness. No claim for refund, damages, or loss shall be entertained.
WAIVER OF LEGAL RECOURSE
By submitting payment, the Client expressly waives the right to initiate or pursue legal action against Jonah Amos or Performance Ready Athletics, LLC in connection with enrollment, billing, cancellation, or service delivery.
ACKNOWLEDGMENT AND ACCEPTANCE
By submitting payment and enrolling in the program, the Client confirms that they have read, understood, and voluntarily accepted the terms of this Agreement. This Agreement shall be governed by the laws of the state in which services are rendered.

LIMIT LATE BOOKINGS
Our team is dedicated to delivering a top-tier, personalized experience tailored to the areas of focus you indicated on the pre-booking form. To ensure we can accommodate your needs and provide in-home massage services in the comfort of your own space, we kindly request that appointments be booked at least two (2) days in advance of your desired date and time.
PAYMENTS
During the booking process, valid card information is required and will be securely stored on file. Please note, your card will not be charged until the service provider confirms and accepts your appointment.

CANCELLATION POLICY
A cancellation fee of $50 will apply to appointments canceled less than 24 hours before the scheduled date and time. This fee also applies to no-shows. By booking an appointment, you agree to these terms and authorize the card on file to be charged for any applicable fees.
